The Australian Government's Support at Home Program officially replaced the Home Care Packages and Short-Term Restorative Care programs on 1 November 2025. For older Australians and their families, this change brings a more streamlined approach to accessing care, equipment, and home modifications, all under a single registered provider.

At Back to Sleep, we work directly with Support at Home providers, prescribers, and care managers to deliver high-quality Assistive Technology (AT) and Home Modifications (HM) under this new program.

The Support at Home Program is an aged care reform designed to give older Australians better access to the services they need to live safely and independently at home for longer. Under the new model, a single registered provider manages each participant's care plan, including the sourcing and delivery of Assistive Technology and Home Modifications.

Assistive Technology under the Support at Home Program includes a wide range of equipment: from pressure care mattresses and hi-lo electric beds through to lift chairs for elderly participants who need support with daily transitions. Home Modifications cover structural changes to the home environment that improve safety, accessibility, and independence.

Funding is allocated across three tiers based on assessed need:

For any Assistive Technology item over $15,000, a Pre-Approval Form is required. Providers upload this form alongside prescriptions and quotes to My Aged Care, and await approval before the item can be purchased under the High Tier funding allocation.

3 ways to manage your NDIS plan

Self-management

The NDIS provides you with funding so you can access the supports that will best help you pursue your goals. If you are self-managed, you pay for your supports directly and claim back from the NDIS to get reimbursed. Remember that when self-managing, you are responsible for being aware of what can be claimed for on your plan.

The NDIS will provide funding in your plan to pay for a Plan Manager who pays your providers for you, helps you keep track of funds and takes care of financial reporting for you.
